When initial appointments are scheduled, new clients will receive an email and are asked to log into our patient portal. Once logged in, new clients are asked to review the following applicable documents which have been provided below for your reference. An electronic signature and submission is required, prior to the first appointment. Time will be allotted at first appointments to further discuss these policies and respond to any questions.
